Explanations:

Professing Christian--This is, in a sense, the beginning of the survey, and your answer to this question is required. For purposes of this survey, you are a Professing Christian if you say you are. I refuse to engage anyone in a theological debate on this issue in conjunction with this survey.
Age--Your age at the time you take the survey. Please note that I have not requested your name, and I promise I won't tell anyone how old you are! Your level of personal and spiritual maturity is influenced by your age, and will also have a bearing on your response to some questions.
Country--The country where you live.
Christian how long?--How long have you been a Christian? Although this is not a required field, your response is requested because your level of personal and spiritual maturity is influenced by your age, and will also have a bearing on your response to some questions.
Organization--This is your church and is needed if your results are to be included in a composite report with that organization. The person leading the class or course will provide you the name to be used.
Group--This is your Sunday School class, Bible study, or other small group, and is needed if your results are to be included in a composite report with that group. The person leading the class or course will provide you the name to be used.
Denomination--A Christian denomination of the church.
Attendance--Include attendance at worship, Sunday School, Bible studies, choir practice or other musical groups, committee meetings, mission projects, etc. Anything that brings you to the church that you attend counts here.
Involvement--Include worship, Sunday School, Bible studies, choir practice or other musical groups, committee meetings, mission projects, etc.
E-mail--Please provide your e-mail address so that I can let you know about and future developments on this survey. Of course, if you are concerned about protecting your anonymity you can leave this off.
